Primary Care

Primary Care incorporates the departments of Internal Medicine/Family Practice, Obstetrics & Gynecology and Pediatrics. These providers will care for your primary health issues and can initiate appropriate referrals to our team of specialists.

Our Primary Care section of our web site shows all of our medical providers, their location and departmental contact phone numbers.

Health Education Center - The Health Education Center is a resource for learning about you. As part of the Health Education Department, the Health Education Center is dedicated to keeping health plan members, employees and the community healthy. Discover a wide range of Health Education Services.

Medicine/Family Practice - Our San Jose Center provides care for those members who are 18 years or older (excluding pregnancy-related care). Outpatient services are divided into adult primary care and adult subspecialty care.

Obstetrics/Gynecology - Our San Jose facility provides routine gynecological and prenatal care. Women are encouraged to select a personal care practitioner in our Ob/Gyn Department in addition to a personal care practitioner in our Medical/Family Practice Department.

Pediatrics - Our San Jose Pediatric Department provides comprehensive medical care, including routine preventive care, immunizations, and acute care services to children from birth through age 18.

Specialty Departments - Your primary care physician will refer you to a specialist when he or she feels that you require specialty care. Some specialty departments, such as our Mental Health and Psychiatry Departments, do not require a referral at all. The specialty departments are referral only.

Physicians who practice in internal medicine, family practice, obstetrics/gynecology, and pediatrics are considered personal physicians. Under the direction of our physician team, registered nurse practitioners are licensed to deliver a broad range of care. They can conduct health assessments, treat common health problems, and provide certain non-prescription medications. They can also serve as your personal practitioner.

If you have an emergency medical condition, call 911 or go to the nearest hospital. When you have an emergency medical condition, we cover emergency care from Plan providers and non-Plan providers anywhere in the world.

An emergency medical condition is (1) a medical or psychiatric condition that manifests itself by acute symptoms of sufficient severity (including severe pain) such that you could reasonably expect the absence of immediate medical attention to result in serious jeopardy to your health or serious impairment or dysfunction of your bodily functions or organs; or (2) when you are in active labor and there isn't enough time for safe transfer to a Plan hospital before delivery, or if transfer poses a threat to your or your unborn child's health and safety.
